Counseling Consent & Cancellation Form

I, the undersigned grant consent to receive Biblical/Christian counseling  with Renewed Mind Inc. I understand the counseling sessions will be 50-60 minutes. All information will be kept confidential unless legal requirement by law.

I also agree to give a 24 hour or better notice for cancellation. I understand if I don't cancel with at least a 24 hour notice I will be charged half price for the missed session.
